As a volunteer of the James McElroy Foundation (JMAC), I commit to
the following:
1. I have read, and will adhere to, the JMAC Volunteer Code of Conduct, and the
JMAC Volunteer Promise. I will perform my volunteer duties to the best of my
ability.
2. I will adhere to the policies, rules, and regulations of JMAC Partners for
whom I perform volunteer duties.
3. I will serve as a volunteer without receiving any monetary compensation or
other financial benefits for my service.
4. I will adhere to your rules and procedures, including record-keeping
requirements and confidentiality of JMAC and client information. In particular,
I confirm that all information I learn about JMAC volunteers, donors, and
clients (including name, address, financial and health information) is strictly
confidential and I agree not to disclose such confidential information.
5. I will I understand that my volunteer work through JMAC and with JMAC
Partners involves risks and I expressly assume all such risks myself,
including, but not limited to, injury or disease associated with my volunteer
work. JMAC shall not be responsible for my security, health, or safety.
6. I, on behalf of myself and my heirs and executors, fully release and forever
discharge JMAC and each of its directors, officers, employees, successors,
agents, representatives and assigns (collectively, the "Released Parties") from
any claims, liabilities, and promises (including without limitation, any costs
and attorneys' fees) of whatever kind, known and unknown, fixed or contingent,
wherever made, and whether or not yet asserted, which I may now or later have
against any of the Released Parties related to my volunteer work with JMAC
which has occurred or may occur after this date. I knowingly, voluntarily, and
intentionally relinquish any right to claims related to my volunteer work with
JMAC which I do not know or suspect to exist in my favor at this date,
including waiving any rights which I may have under California civil Code
Section 1542, which states:
"A general release does not extend to claims which the creditor does not know
or suspect to exist in his favor at the time of executing the release, which if
known by him must have materially affected his settlement with the debtor."
7. I understand that I or JMAC can end my volunteer status at any time, with or
without cause, by giving notice to the other.
